I understand that the minor child will be transported by Waterford Montessori Academy’s bus. I understand that every reasonable effort will be made regarding safety of the transportation and the safety of the trip. Despite the fact that every reasonable effort is being made to secure the safety of the minor child, accidents do happen. In that regard, I, as the parent/guardian of the minor child and on behalf of the minor child, to the extent permitted by law, unconditionally release and discharge Waterford Montessori Academy, its Board of Directors, and its management company and employees, from any claims, disputes, claims of ordinary negligence, judgments, personal injury claims, and/or administrative actions that may occur as a result of the minor child’s participation in the trip and transportation to and from the trip. This release and discharge does apply to acts of ordinary negligence but does not apply to acts of gross negligence.
As parent/guardian of the minor child, I have reviewed this Waiver and Release and by completing the information below represents my acceptance of the terms and conditions set forth herein.
I consent to my child being transported for this trip by Waterford Montessori Academy driven by a licensed bus driver.
